1. In 100 words or less, please provide a summary of the organisation you are nominating and what it does. Include in this when the organisation was founded, who it serves and how.



2. Please give an outline of the organisation's objectives.
This information will be used to judge the clarity of the organisation’s objectives, and how a relevant strategy has been created to meet them.






3. Please provide evidence of how these objectives are being met, and how the foundations of sound management are in place for continued success in future.
This should included a description of the impact the organisation has had since its foundation. Can it be shown to have achieved both awareness of itself and its causes and set the foundations of excellent management and accountability? Has it identified any unmet needs and how has it addressed these needs?

Excellence may take the form of identifying a need, management, planning, execution, commitment or innovation. This should provide evidence of how your organisation displays excellence in any or all of these areas.

Please detail the overall impact that the organisation has had. In this section evidence may be quantitative, qualitative or a mixture of both.
